Pediatric Glaucoma Surgeries

Laser Treatment in Pediatric Glaucoma

In childhood glaucoma, laser treatments may be used in limited and selected situations depending on disease type and ocular structure. The aim is to help reduce intraocular pressure and prevent optic nerve damage. In pediatric patients, treatment planning is based on age, corneal status, intraocular pressure, and visual development as a whole.

Goniotomy

Goniotomy is a microsurgical procedure targeting the eye’s internal drainage angle, particularly in congenital and childhood glaucoma. It aims to open the structures that obstruct aqueous outflow and reduce intraocular pressure. Early diagnosis and appropriate surgical timing are critical for visual development in children.

New-Generation Trabeculotomy

Trabeculotomy is a surgical procedure targeting the drainage canal to enhance aqueous outflow in childhood glaucoma. New-generation techniques, such as 360-degree suture trabeculotomy, aim to treat a wider portion of the drainage angle for more effective pressure control. The appropriate method is selected based on the child’s age, corneal clarity, and glaucoma severity.

Pediatric GATT Surgery

Pediatric GATT is a modern surgical technique that opens the eye’s natural drainage canal from within in childhood glaucoma. It may improve aqueous outflow without requiring an external incision. It is not suitable for every patient, and the decision is made based on detailed examination findings and disease characteristics. It may have higher success rates than conventional goniotomy in selected cases with relatively clear corneas.

Pediatric Trabeculectomy

Pediatric trabeculectomy is a glaucoma surgery that creates a new drainage pathway to lower intraocular pressure in children. It is typically considered when angle surgeries are insufficient or in advanced cases. Because wound healing responses differ in children, close postoperative follow-up is essential.

Pediatric Glaucoma Drainage Device Implantation

In childhood glaucoma, drainage implants may be used when intraocular pressure cannot be adequately controlled by other methods. Devices such as the Ahmed valve or Paul implant allow controlled aqueous drainage. These are highly specialized procedures that should be performed in experienced centers and require long-term follow-up.

Pediatric Complex Glaucoma Surgeries

Pediatric complex glaucoma surgeries include individualized procedures for children with congenital anomalies, previous surgeries, corneal abnormalities, or advanced optic nerve damage due to elevated intraocular pressure. The goal is not only to reduce intraocular pressure but also to preserve visual development. Therefore, follow-up is as important as the surgical procedure itself.
